Please complete the initial intake paperwork prior to the first appointment. Completing this paperwork beforehand will allow us to do business efficiently when we first meet. Rather than wading through a lot of paperwork together, we will be able to move quickly into the more therapeutic part of our initial session.
If you have access to a printer, just click on the links, print & complete all forms, and bring them with you to our first session.
If you cannot print the forms, no worries. Call the office to have a packet either mailed to you or schedule to arrive a half hour early, to fill them out in the waiting area. I look forward to meeting you!
